I/We hereby grant Weatherly Road Assembly of God permission to the rights of the Child’s image, likeness, and sound of their voice as recorded on audio or videotape without payment or any other consideration. I/We understand that the Child’s image may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein the Child’s likeness appears. Additionally, I/We waive any right to royalties or other compensation arising or related to the use of the Child’s image or recording.
I/We agree that Weatherly Road Assembly of God may use such images of the Child with or without the Child’s name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and web content.
There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
I/We acknowledge that I/We have completely read and fully understand the above release and agree to be bound thereby. I/We hereby release, defend, hold harmless and indemnify Weatherly Road Assembly of God from any and all claims for utilizing this material.

I, the undersigned parent/guardian, do hereby grant permission for my son/daughter, named above, to attend the Egg Hunt. In order that my child may receive the proper medical treatment in the event that he/she may sustain injury or illness during the Ehh Hunt, I hereby authorize the church staff to obtain or provide medical treatment for my child for such injury or illness during this event, and I hereby hold the church staff and sponsoring organization(s), as well as its representatives, harmless in the exercise of this authority.
I further understand that there is always a possibility that my child may sustain physical illness or injury while at the Egg Hunt. If this occurs, I hereby authorize the church staff and representatives to refer my child to a medical treatment center, (hospital, etc.). I further acknowledge and understand that I will be responsible for any medical bills that might may be incurred on behalf of my son/daughter for physical illness or injury that he/she may sustain during the Egg Hunt.
Understanding that there is always a possibility that my child may sustain physical illness or injury, I acknowledge and understand that my child is assuming risk of such physical illness or injury by his/her participation, and I further release the sponsoring organization and its representatives from any claims for personal illness or injury that my child may sustain during the camp. I further acknowledge and understand that my child will be responsible for his/her failure to abide by the rules and regulations of the Egg Hunt.
